Your Complete CSEMS Application Packet Should Include:

1. CSEMS Application Form
2. Your Personal Statement (see below for details)
3. A Letter of Recommendation from a faculty member nominating you for the CSEMS Scholarship
4. A current copy of your Student Academic Progress Report (SAPR) and SAT or ACT score (for Senior High School student applicants) or an informal copy of your Community College transcripts (for Community College transfer student applicants).
